While the 2024 Form 1095C provides information about medical insurance offer and coverage for employees in 2024, these forms are not required to complete taxes.

If desired, a 2024 Form 1095C may be prepared and available within 30 days of request. To request a copy of a 2024 Form 1095C, please email eharasim@saintfrancischurch.org, call 515-223-4577, or mail your request to 7075 Ashworth Rd., West Des Moines, IA 50266.
