Medical Forms

The following forms must be on file before the start of the school year:

Kindergarten/New 1st-8th grade Students:

Certificate of Dental Screening
Immunization Record
Student Physical Form
Vision Screening

3rd Grade – Vision Screening Form

4th Grade – Current Physical (must be within the last 9 months)

7th Grade – Current Physical (must be within the last 9 months)

OTHER FORMS:

Current Athletic Physical – needed each year before your middle school student is able to play any school sport.
Immunization Exemption Form (Religious and Medical)
Medication Release (office must have copy to give student any medication during the school day)
